Headquartered in Venice, FL, we give you options and treatments to fight your acute illnesses and injuries in the office or from the comfort of your own home. Skip the long waits and potential exposure to illness in our small practice. We are licensed in Florida, and Travis is also licensed in Iowa, serving you remotely. Our pricing is transparent and up front. We value your privacy, so we are out of network with insurance, except for straight Medicare. We work hard to keep your prices affordable and have no membership fees. Simply pay for the care you receive and nothing else. Many health insurances will reimburse you for your visit at out-of-network rates, which generally nearly cover your costs.
Our Nurse Practitioner, Travis Prachar, has been practicing in nursing since 1993. She earned her Master’s of Nursing/Family Practice in 2000 and has been practicing as a Nurse Practitioner since 2000. She has extensive experience in pediatrics, home health, cardiology, community health clinics, primary care, pulmonology, cardiothoracic surgery, and urgent care.
To set up an appointment, please click below. Please be aware that pretreatment consent and health history forms MUST be filled out before the start of your appointment. These will be emailed directly to you at the moment you book the visit. Look for an email from “Elation” health services, fill out all forms, and submit them online. Please also choose to OPT IN to text messaging by texting the word “START” to 36331, as this is the most efficient way to send the link for telehealth visits. In order for you to receive a copy of your visit, you will also need to sign up for our patient portal called “Passport”. This will enable easy communication between us, allow you to print any needed work/school notes or release, and see the details of your care plan. You will receive a request for payment at the end of your visit.
As a cooperative healthcare office, acupuncture and herbal medicine services are also available.
